Annual GPAC Meeting Summary MS/PhD Graduate student progress toward degree requirements is assessed each year at the annual graduate committee GPAC meeting. Summary notes are entered and the report is shared with the student. Return to Graduate Coordinator within 2 weeks of annual meeting.

Report of MS Examination MS Candidates for the MS degree must present this completed form to the Examination Committee at the time of the MS Exam. Signed report to be turned in to Graduate Coordinator.

Examination for Admission
for Ph.D. Program without
an M.S. Degree
MS Currently, only students holding an M.S. degree can be admitted to the Ph.D. program.  This requirement can be “by-passed” if 1) the student is primary author on a manuscript submitted to a peer-review journal for publication, and 2) the advisor consents to the student proceeding into the Ph.D. without an M.S.  A committee of 3 faculty (including advisor) attends a presentation and defense of the research by the student (similar to an M.S. defense), and a decision is rendered. Completed form to be turned in to Graduate Coordinator.

Approval for Preliminary PhD Examination PhD The preliminary examination can be scheduled taken after all certified coursework is completed (except one graded FS 900 “exit” seminar).  This form accompanies the “prelim” for the examination committee to ensure completion of certified coursework.  Successful completion of the prelim confers “dissertator” status.

PhD Final Oral Examination
PhD This form must be completed prior to taking the final PhD exam and should be completed at the same time the Graduate School’s PhD Final Oral Exam Committee Form is processed.

WARRANT TITLE LEVEL NOTES
Masters Degree Warrant
Request Form
MS Warrants must be requested a minimum of three weeks prior to scheduled defense/exam or the degree deadlines.
        1.   Schedule conference room for defense with Graduate
          Coordinator*.
    2.   Complete Warrant Request Form (see Graduate Coordinator).
    3.   Pick up Warrant from Graduate Coordinator (this could take a few weeks                     since the Graduate School generates this document).
    4.   Complete Report of MS Examination Form.
    5.   Submit completed Report of MS Examination Form and signed MS Warrant                 copy to Graduate Coordinator.
Request for
Preliminary Warrant
PhD Warrants must be requested a minimum of three weeks prior to scheduled defense/exam or the degree deadlines.
        1.   Schedule conference room for prelim with Graduate Coordinator*
    2.   Complete Prelim Warrant Request Form (see Graduate Coordinator).
    3.   Pick up Prelim Warrant from Graduate Coordinator (this could take a few                     weeks since the Graduate School generates this document).
    4.   Complete Report of Approval for Preliminary PhD Examination Form.
    5.   Submit completed Approval for Preliminary PhD Examination Form and                       signed Prelim Warrant copy to Graduate Coordinator.
PhD Final Oral Committee Approval Form PhD Warrants must be requested a minimum of three weeks prior to scheduled defense/exam or the degree deadlines.
        1.   Schedule conference room for final PhD oral examination with Graduate                       Coordinator*
    2.   Complete PhD Final Oral Committee Approval Form (see Graduate                                   Coordinator).
    3.   Pick up PhD Warrant from Graduate Coordinator (this could take a few weeks             since the Graduate School generates this document).
    4.   Complete Approval for and report of PhD Oral Examination.
    5.   Submit completed Approval for and report of PhD Oral Examination and                     signed PhD Warrant copy to Graduate Coordinator.

*Please note that Babcock Rm. 201 is scheduled for many events, please plan ahead to secure this resource.

**Important Note: The "Window Period" is the time between the end of one degree period and the beginning of the next.
You must have been registered for the previous semester (Fall, Spring, or Summer). If all degree requirements are met by the end of the window period, your degree will be granted for the following semester. However, you will not have to register or pay fees for the next semester.
